We recommend that you:

1. save it to your desktop, 
2. fill it in and save the form under a new name for each item (you only need to fill in the 'artist statement' section once), then
3. email each form with a 'jpeg' image file of each item you are submitting to info@artshape.org.uk

Call the office on 01452 863855 to request confirmation that your submitted items have been received.
